Setting Health Care Goals When You Have Health Problems

Do you have a serious illness or are you at risk of developing one? If so, it’s important to think ahead about what would matter to you most if you got sicker. Different people want different things, and what matters to you should guide decisions about your treatment plan. When you have a serious health problem, some of those decisions might be tough. The information in this booklet can help you talk with your health care provider and your family about what matters most to you. This will help you receive the services and treatments that best match your own values and preferences. This booklet also offers information about CPR, mechanical ventilation, feeding tubes, and dialysis.
